no, it'd be easy even 
hair is the biggest problem, but the only fluff seen has a black background, so you can blur completely around it.
It's really sad that the collar isn't cardinal-red
it's pointless to photoshop it red tho, as the clou gets lost.
compare this one, the hair on the arm at the right side was critical.
yet it's not too difficult to blur the background.
the more the background directly behind the hair is equally colored the more easy it is, you just don't include the hair in the selection, as blurring plain collors wouldn't make sense.
True, mostly I have to blur several times with incresing amount while shrinking the 'selection' some pixels, to avoid visible artefacts on the border of the processing.
She lost some hair tho
Most of the work however usually goes to 'selecting' different levels of blurring for the different 'layers in depth' in the picture.
And you can't do it in one shot, you have to adjust the selection a few times, each time a layer.
Happily the second blurred 'layer' already profits from the blurring of the first blurred layer, as there's less need for precision, and so on.
Nothing of all this in the pope picture tho.

Hm, I added the smoke by myself to get some extra life in it, but it's completely nonsens if you know that one is supposed to inhale and not to blow
